For the purpose of contacting you about your counselling sessions, I collect:

Your address
Your phone number
Your next of kin
Your email address
I am registered as the Data Controller for my business with the Information Commissioners Office (ICO) and take the protection of your data very seriously. I store your contact details on my electronic devices which are password protected. It is a requirement of my insurance that all personal data and notes are retained for a period of 7 years, after which they are destroyed. You have a right at any time to ask what information I hold on you.
If you feel there has been a breach of your data and wish to raise a complaint you can contact me directly in the first instance.
If my responses are not acceptable to you, you can contact the Information Commissioners Office at ico.org.uk or on 0303 123 11 13.

In the event of my death or incapacitation through illness, a nominated person will receive the name and contact details of my current clients. The nominated person will contact my clients to inform them of my situation.
