Privacy Policy

Susan Kay collects basic personal data about you.

For the purpose of contacting you about your counselling sessions, I collect:

Your name, address, phone number/s, email address, emergency contact details and GP details.

I may also retain the following records; Assessment and intake forms, sessions notes, correspondence between us and payment records.


I am registered as the Data Controller for my business with the Information Commissioners Office (ICO) and take the protection of your data very seriously. I store your contact details in paper form only. They are stored in a locked filing cabinet at my home address in secure premises. Access to theses records is strictly restricted to myself, Susan Kay, and my appointed Clinical Executor (in the event of my death or incapacity).

It is a requirement of my insurance that all personal data and notes are retained for a period of 7 years, after which they are destroyed. You have a right at any time to ask what information I hold on you.

If you feel there has been a breach of your data and wish to raise a complaint you can contact me directly in the first instance.

If my responses are not acceptable to you, you can contact the Information Commissioners Office at ico.org.uk or on 0303 123 11 13.

LIVING THERAPEUTIC WILL

In the event of my death, or incapacitation through illness, my Clinical Executor will receive the name and contact details of my current clients. The nominated person will contact my clients to inform them of my situation.
